Faculty/Staff Name Change

If you have changed your name due to marriage or divorce, please complete the following steps:

1) Human Resources: The employee will need to complete the Name Change in OneUSG Connect. The name entered must match the name on the Social Security Card exactly. Instructions for entering a name change in OneUSG Connect can be found here. Name Change

Employee will need to bring a copy of the new Social Security card to HR. Once received, HR will approve the Name Change in OneUSG Connect and Banner and then forward to Accounts Management.

2) Accounts Management: Human Resources will notify us after they have updated OneUSG & Banner.  If you want your email account renamed to match your new legal name, please complete the following change form:
Name Change Form
     Account Request Type – Faculty/Staff Change Account
     Click Submit
     Type of Request – Email Account Rename
     Click Submit
